Additional research may be necessary to complete those requirements.   Other examples include but is not limited to a compliance plan, security plan; HIPAA policies: equipment certifications, etc.

City of Grand Junction
City Tax report - Due annually on February 28. Visit website

Mesa County Taxes –Due annually on April 30 or February 28 and June 15. Visit website

State of Colorado

  • Secretary of State filing – Yearly corporations are required to submit a report to the Secretary of state to show that they are in good standing.  This report is due each year at the time of the original incorporation date.  For further information please visit Periodic Report Visit website

  • CAQH - Physician credentialing system - Visit website
  • NPPES updates - This registry enables you to search for a provider's NPPES information, update organization and provider profiles, etc. - Visit website

CLIA certification (lab certification for offices) – How to apply for CLIA Certificate, Categorizations of test, Certificates of Waivers, etc.    Inspections of your lab are every other year - Visit website

  • CMS Enterprise Portal –Is a gateway to all of the information and links you will need to get started with Medicare (PECOS & NPI), participate in PQRS, Meaningful Use (If you have not already done so), EIDM to access QRUR systems. Visit website

Malpractice requirements

  • Signature logs – Create each year with Physician and medical staff signature and initials

Termination of provider
Each insurance carrier must be notified 30-90 days prior to leaving; check your contract for specific detail.
