I-9 Form:  U.S. Department of Homeland Security form for Verifying Eligibility for Employment.  Employers are responsible for completing Form I-9 to document verification of the identity and employment authorization of each new employee (both citizen and non-citizen). 
Section 1. Employee Information and Attestation should be completed and signed by new employees no later than the first day of employment, but should NOT be completed before the employee has accepted the job offer.
Section 2: Employer review and verification must be completed by examining evidence of identity and employment authorization within 3 business days of employee’s first day of employment. Open form

This form is not filed with any government agency.  Form I-9 must be retained by the employer and made available for inspection by U.S. Government officials.  Form I-9 must be kept in a separate file outside of employee personnel files.  The form may be retained electronically.
Form I-9 and copies of presented documentation must be retained for the entire term of employment.  Once employment ends, the form must be retained for either 3 years after the date of hire or 1 year after the date employment ended, whichever is later.

Obtain a Background check:  Obtain a signed authorization to perform a consumer credit report and background check. 

W-4 Form:  Must be completed by employees to notify employer of withholding amounts for Federal Income Tax. Open form

W-9 Form: Must be completed by all independent contractors receiving payment for services from the organization. Open form

Application for Employment: to be completed by all employees applying for posted position. Open form

Employee Handbook and acknowledgment of Receipt of handbook:  All new employees must be given a copy of the employee handbook, (electronic or paper copy).  Employees must sign a dated document confirming receipt and review of the employee handbook.

Confidentiality Statement: Signed document acknowledging training on organizational privacy policy and confidentiality policies and procedures.
